Abstract
Sustainment of innovative health systems science (HSS) programs presents planning, assessment, and evaluation challenges. This chapter reviews key features of sustainability and tools for continuously improving and sustaining programs. The chapter takes a long-term view on sustainability by focusing on goal setting, aligning assessments with outcomes, developing program evaluation systems, using improvement processes, and highlighting common opportunities for and barriers to success. By applying models of assessment, evaluation, and continuous quality improvement as well as the principles of long-term sustainability, institutions will be able to continue integration and improvement of their HSS programs well into the future.
